The exposure of subcutaneous tissue following a loss of skin integrity provides a moist, warm and nutrient-rich environment for microbial colonisation and proliferation. Accordingly, the use of tropical antimicrobial agents is crucial to the management of surgical site infections (SSIs), as well as chronic wounds such as venous statis ulcers, diabetic foot and pressure ulcers, burns and fungating lesions.
By virtue of their mode of action, hypochlorous acid (HOCl) cleansers offer a safe, effective and potentially cost-effective solution to managing bioburden, reducing healing times in chronic and septic wounds, and – most importantly – improving the quality of life for patients with chronic wounds.

HOCl is a natural part of our body’s innate physiological immune and/or inflammatory response, and is generated inside phagocytic white blood cells.
HOCl wound cleansers:
- Are a weak acid and oxidising agent
- React more rapidly to organic matter than other chlorine-based disinfectants
- Are better able to penetrate microbial cell walls due to having no charge and a low molecular weight
- Are reactive when stabilised, but are not persistent (unlike sodium hypochlorite)
- Are classified as non-hazardous due to their low pH and available chlorine concentration
- Do not require specialised PPE or environmental disposal
- Are non-corrosive and compatible with inanimate surfaces and environmental materials
Hypochlorous acid wound cleansers are chlorine-based, chlorine-releasing agents, and were approved by the FDA for the treatment of wounds in 2016. Their antimicrobial efficacy arises from the freely available chlorine species present and/or formed within the solution on contact with water as well organic and inorganic material. The main active chlorine species are hypochlorous acid (HOCl) and hypochlorite (OCl), so it is difficult to maintain the stability of a hypochlorous acid solution.
At approximately pH 7.5, the total chlorine species in the solution is a 50/50 mix of hypochlorous acid and hypochlorite. 2 This means that the potency, stability and safety of HOCl preparations depend on the purity of the solution, optimal pH, and the avoidance of contaminating molecular species of aqueous chlorine (such as hypochlorite and elemental chlorine). 1, 3, 4
Several independent studies show that HOCl disrupts microbial biofilm, exerting broad-spectrum antimicrobial properties against clinically important bacteria (including spore-forming species), prions, viruses and fungi. 3, 4, 7, 8 It targets multiple sites on or within microbial cells, contributing to its broad-spectrum activity, but does not induce genetic mutation and antimicrobial resistance. 3
HOCl cleansers are ideally suited for moistening, cleansing and debriding wounds (with at least 3-5 minutes of contact time). Utilised this way, HOCl cleansers are effective for:
- Physical disruption of microbial biofilms
- Control of bioburden and malodour

- Wound bed preparation (loosening of superficial debris and autolytic debridement)
- Maintenance of optimal wound bed pH
